The fallout from the dramatic U.S.-Israeli strikes on Iran continues to unfold, as Iranian state media reports that the wife of Supreme Leader Ayatollah Ali Khamenei has died from injuries sustained in the attack that killed her husband.
According to Iran’s state television outlet Press TV, Mansoureh Khojasteh, 78, was wounded during Saturday’s coordinated military operation and later succumbed to her injuries. Reports indicate she had been in a coma since the strikes. Khojasteh and Khamenei had been married since 1964.
Her death adds another layer of gravity to an already explosive weekend that has reshaped the political landscape of the Middle East.
A Weekend of Escalation
The joint U.S.-Israeli operation targeted key Iranian leadership following weeks of mounting tensions. On Saturday, President Trump announced that Khamenei had been killed in the strikes, describing the mission as a decisive blow against a longtime adversary.
“Khamenei… is dead,” Trump said in a social media post, framing the operation as justice for Americans and others who he said had suffered under the Iranian leader’s rule.
Iran quickly retaliated, launching attacks against Israel and several Gulf states, including Saudi Arabia. The region now faces a rapidly widening conflict, with global leaders watching closely.

U.S. Military Casualties Rise
As the situation intensifies, the human cost is also mounting. U.S. Central Command confirmed Monday that a fourth American service member has died from injuries sustained during the operation. Three other U.S. troops were previously reported killed in action.
The latest casualty had been seriously wounded during Iran’s initial counterattacks and later died from those injuries, according to Centcom.
President Trump acknowledged the risks of the unfolding conflict, saying additional American casualties were “likely” before the fighting subsides. In a video message, he offered prayers for the wounded and condolences to the families of fallen service members.
“Sadly, there will likely be more,” he said. “That’s the way it is.”

Khojasteh’s reported death underscores how deeply the strikes have reverberated within Iran’s leadership circle. While much of the world’s focus has been on the geopolitical implications, the personal losses on all sides are becoming increasingly visible.
